Care services

Our service provides short breaks for adult carers. Our fully trained Carer Support Workers come into your home, step into your shoes and give you a break from the day-to-day stresses of caring. Services are arranged around your needs and can be for a few hours, a whole day, overnight or longer. Carer Support Workers can provide a service in your home or take the person you support out into the community. We're not restricted by age, health condition or personal circumstances. The families that we support all have very different requirements related to their health or ability.

We’re flexible – it’s about what you need to help you in your caring role.

Our Carer Support Workers are trained to at least NVQ Level 2 in Health and Social Care and have all been vetted through the Disclosure and Barring System (DBS). They provide practical, personal and specialised care, allowing you time for a much needed break. 

Our service is registered with the Care Quality Commission, ensuring that it is delivered to a high standard. 

 

What You Can Expect From Us

Before we provide a service, a Care Co-ordinator will visit you at home to meet you and the person you care for. This will help them to understand your needs and how the service can best support you. If we can help, the Care Co-ordinator will carry out a Care Needs Assessment to identify the care and support needs of the person you care for and agree with you both how and when the service will take place. 

Your Care Plan

The Care Co-ordinator will create a care plan that will explain the tasks that you would like a Carer Support Worker to carry out while you have a break. This can include any personal care and administering prescribed medication. All agreed tasks will be risk assessed to ensure the safety of the person you care for and the member of staff involved.  The Carer Support Worker will use the care plan as a guide at each visit and use it to record the services they provide.

When Your Service Starts

Once the care plan is completed the Care Co-ordinator will contact you to arrange a start date and let you know who your regular Carer Support Worker will be. We understand that it can be disruptive or even distressing for families if appointment times or workers are not consistent. We will do our best to ensure that your service times are when you want them and that you see the same Carer Support Worker each time. If your Carer Support Worker cannot visit, due to holidays or sickness, we’ll contact you as soon as possible to let you know and arrange a replacement service.

If there are any issues or changes to your service that you want to discuss, a Care Co-ordinator will always be available during office hours, Monday to Friday. At any other time, we have an ‘on-call’ phone system that is staffed 24 hours a day, every day of the year.

Reviewing Your Service

Once your service has begun, we’ll get in touch after 6 and 12 weeks to check that everything is running smoothly. Each year, we’ll also carry out a review and re-assessment to ensure that the service continues to meet your needs. We encourage carers to contact us at any time, so there’s no need to wait for a review if there’s an aspect of your service that you want to discuss.
